This nail is overdriven, and has punched a hole right through the shingle.

This nail has also missed the nailing line completely.
Here are examples of both staples and nails, not properly placed. A staple or nail directly in a keyway such as these, have a high probability of causing a leaks.
This is what composition shingles look like when they are burned out. In this case only the ridge burned out, because this Homeowner did not have adequate attic ventillation, it baked the ridge shingles from the inside out.
The black area is actually burnt shingles. They were baked from the inside, being installed on a roof with no ventilation.
This is an example of what a repair job should not look like. When you have vents or other items on the roof repaired, the work should match the existing roofing.

If you see waves like this in your roof, you have got dry rotted sheathing. There is a cathedral ceiling below, without adequate ventillation. That is what caused this Homeowner's roofing substrate to rot, and create a potentially dangerous situation.
Here the shingles have been removed, the bad wood replaced, new felt and shingles installed, and a ridge vent added to prevent future rotting.
Someone had no idea how to install a solar tube, but tried it anyway. The damage was minimal, but still there were ceiling repairs necessary.

The integrated rubber portion of this "Oatey" brand flashing has dried and fallen apart, allowing water to run down the pipe and leak inside the wall.
